If you determine that mentoring is a service for you please complete the questions below. The purpose of the questionnaire is to provide the mentors with insight on the individuals past, present and future goals. After the questionnaire is returned, you will be scheduled for three, 50 min sessions per month. These sessions will be scheduled two weeks in advance and held approximately on the same day and time each month, so that they can be continuing sessions.
